Breast size and shape are important, so be honest and open about your expectations when talking with your surgeon. Implant type and size will be determined not just on your desired increase in size but more importantly on your breast anatomy, skin elasticity and body type.
Saline implants are filled with sterile salt water. They can be filled with varying amounts of saline which can affect the shape, firmness and feel of the breast. Should the implant shell leak, a saline implant will collapse and the saline will be absorbed and naturally expelled by the body.
Silicone implants are filled with an elastic gel. The gel feels and moves much like natural breast tissue. If the implant leaks, the gel may remain within the implant shell, or may escape into the breast implant pocket.

Septoplasty is a surgical procedure to correct the shape of the septum of the nose. The goal of this procedure is to correct defects or deformities of the septum. The nasal septum is the separation between the two nostrils. In adults, the septum is composed partly of cartilage and partly of bone. Septal deviations are either congenital (present from birth) or develop as a result of an injury. Most people with deviated septa do not develop symptoms. It is typically only the most severely deformed septa that produce significant symptoms and require surgical intervention. However, many septoplasties are performed during rhinoplasty procedures, which are most often performed for cosmetic purposes.

Septoplasty is performed to correct a crooked (deviated) or dislocated septum, often as part of plastic surgery of the nose (rhinoplasty). The nasal septum has three functions: to support the nose, to regulate air flow, and to support the mucous membranes (mucosa) of the nose. Septoplasty is done to correct the shape of the nose caused by a deformed septum or correct deregulated airflow caused by a deviated septum. Septoplasty is often needed when the patient is having an operation to reduce the size of the nose (reductive rhinoplasty), because this operation usually reduces the amount of breathing space in the nose.

Nasal obstruction is one of the most common problems that brings a patient into a physician’s office, and septal deviation is a frequent structural etiology. As a result, surgical correction of septal deviation is the third most common head and neck procedure in the United States, and it generally is performed to improve quality of life. Presently, a variety of additional indications exists for septoplasty, from intractable epistaxis to harvesting cartilage for use in rhinoplasty.

The nose plays a major role in facial aesthetics. Simply narrowing the bridge, perfecting the tip, and removing ridges through rhinoplasty can improve your appearance and enhance surrounding features. During the nose reshaping procedure, surgeons use the open or closed rhinoplasty technique to reshape nasal cartilage so the nose blends in with surrounding features. The open approach involves the creation of an external incision across the columella; in the closed technique, surgeons perform surgery through incisons that are made in the nostrils. The benefit of the open rhinoplasty method is that it provides the surgeon unrestricted access to nasal cartilage and bone.

The transumbilical breast augmentation technique, or TUBA, is unique in allowing the Dr. David Kim to insert the implants through a small incision in the navel rather than in the underside of the breast, under the arm or around the nipple. Currently, only saline implants can be placed using the TUBA technique. The implants are still deflated when inserted; they are moved into position under the muscle behind the breasts, and then filled with saline (salt water solution) to the predetermined volume.

All incisions can be used successfully for a breast augmentation. Which incision is best for you will depend on your particular anatomy combined with your preference in location as well as the comfort level of your surgeon for each particular procedure. Dr. Kim has performed hundreds of procedures using all the different incisions so it will come down to your desires and what will be best for you based on your anatomy.

Rhinoplasty, or nose surgery, is one of the most common of all plastic surgery procedures and is commonly in the top three of all cosmetic surgeries. Rhinoplasty can reduce or increase the size of your nose, change the shape of the tip or the bridge, narrow the span of the nostrils, or change the angle between your nose and your upper lip. It may also correct a birth defect or injury, or help relieve some breathing problems.

If you’re considering rhinoplasty, this information will give you a basic understanding of the procedure-when it can help, how it’s performed, and what results you can expect. It can’t answer all of your questions, since a lot depends on the individual patient and the surgeon. Please ask Dr. Kim about anything you don’t understand.

I am from out of town and I traveled in from North Carolina to Beverly Hills California to have Dr. David Kim perform my breast augmentation. A breast lift is not a simple operation, but it’s normally safe when performed by a qualified plastic surgeon. Nevertheless, as with any surgery, there is always a possibility of complications or a reaction to the anesthesia. Bleeding and infection following a breast lift are uncommon, but they can cause scars to widen. You can reduce your risks by closely following Dr. Kim’s advice both before and after surgery.
Mastopexy does leave noticeable, permanent scars, although they’ll be covered by most bras or bathing suits. (Poor healing and wider scars are more common in smokers.) The procedure can also leave you with unevenly positioned nipples, or a permanent loss of feeling in your nipples or breasts.
Mastopexy usually takes one and a half to three and a half hours. Techniques vary, but the most common procedure involves an anchor-shaped incision following the natural contour of the breast. The incision outlines the area from which breast skin will be removed and defines the new location for the nipple. When the excess skin has been removed, the nipple and areola are moved to the higher position. The skin surrounding the areola is then brought down and together to reshape the breast. Stitches are usually located around the areola, in a vertical line extending downwards from the nipple area, and along the lower crease of the breast.
Some patients, especially those with relatively small breasts and minimal sagging, may be candidates for modified procedures requiring less extensive incisions. One such procedure is the “doughnut (or concentric) mastopexy,” in which circular incisions are made around the areola, and a doughnut-shaped area of skin is removed. This is also known as a circumareolar lift.
If you’re having an implant inserted along with your breast lift, it will be placed in a pocket directly under the breast tissue, or deeper, under the muscle of the chest wall.
